Most likely, at some point you checked the "do not ask me again" checkbox without realizing you'd done it. This will result in not having a confirmation before items are purchased. To get this dialog back, go to your iTunes store account (select view my account from the store menu) and go to the bottom. Click the "reset all warnings" link to get all confirmation dialogs back.

That's interesting, I have always received a dialog asking if I'm sure. What you should do is contact the iTunes store via the feedback or when you receive the e-mail for your purchase, there is a link in the message you can use to contact them for any problems.

The major difference between a thing that might go wrong and a thing that cannot possibly go wrong is that when a thing that cannot possibly go wrong goes wrong it usually turns out to be impossible to get at or repair.
