That sounds like a smart idea, but I don't trust automatic "smart"
procedures that much.

As an example, I have a lot of documents that have the same first
heading (because they are created from the same template), something
like "$(TEAMNAME) Meeting Minutes", they are differentiated by a text
field below that contains the date.

I don't doubt that, but obviously the file name (or the title) the user
has chosen was good enough for him, showing something else in the
caption bar is like treating him like a child.

Your suggestion could be a good one for the creation of a file name or a
title as a *proposal* when the user is going to enter it.
