Technically, it would be possible to get use the
same trigger that adds a firewall rule to let the
user through the gateway when he authenticates to setup a CBQ rule for it.
Would it be difficult to implement within pfsense?

Not unless m0n0wall does it. All of the captive portal magic is
happening in the m0n0wall developers circle currently.
