I have already explained in my previous email that there is no clearly 
identified .xsl file which would allow you to implement the feature you 
want.

Then, if you are really interested in having both an automatically 
generated index and a back of the book page as explained below, you'll 
have to convert your ditamap to a bookmap (by hand).
