I don't think it is possible to achieve this without customizing the 
XSLT stylesheet.

However, it's probably not difficult to enhance ditac (i.e. 
http://www.xmlmind.com/ditac/_distrib/doc/manual/xsltParams.html#xsltParams__header-left)
 
to allow this. Therefore we'll probably implement this feature in next 
release[*].

For now, the first page of a chapter has no %chapter-title%, which is 
partly what you want.

However, you are right: this is indeed an omission. We'll implement this 
feature for headers and/or for footers in next release[*].
