Most authors put <indexterm> entries in the body of the topic rather 
than in the prolog. So basically, you are saying that the most common 
case does not work.

XSLT stylesheet customization allows to customize the ``looks'' of the 
output of ditac.

What you describe above clearly does not seem related to the looks of 
the output of ditac.

Therefore this seems to be an issue.

However we have *dozens* of tests specifically related to Index 
generation and this issue never, ever, happens in these tests.

So please explain how to reproduce this issue.

Before doing that, even if it is unlikely that this issue is caused by 
your customization layer, I would suggest that you try converting your 
document to .docx without your customization layer and see what happens.

Even easier, try to convert your document to PDF (i.e. simply select 
ditaToPDF instead of ditaToDocx) and see what happens.
