I think this is a known issue with Microsoft's standard .chm viewer. If my memory serves me well, I always have this problem when I generate the .chm file on a network drive/shared folder (e.g. z:). When I copy the .chm file to a local drive (e.g. c:) and then reopen the .chm file in Microsoft's standard .chm viewer, then this problem goes away.
