Just for another data point...
We have had to duplicate a pre-existing title page format by hand-crafting
the title page layout with FO. In order to achieve the (minor amount) of
customization that Jirka refers to, we use entity definitions. Specifically,
we have a title-page FO template and a simple script that adds the
the entity definitions at the top as part of processing steps before
invoking DITAC.
