By the version number of the files already installed. So, AnApplication-1.0.2 to AnApplication-1.0.5, apt-get upgrade will upgrade (So, will dist-upgrade), but apt-get upgrade won't upgrade 1.0.x to 1.1.y, but dist-upgrade will.
