Paul. I think actually that we need to start maintaining a list of things that ought to be explicitly looked for when reviewing documents for publication, and this sort of thing would be one such. This is not to say that it's not a hard problem, though--such a list could easily become unwieldy. We tried to do something like that with RFC 7227 section 13. The problem is that if there isn't a single resource to go to for this stuff, and there's no step in the review process where this sort of check happens.
