Hi Eric,
I'm not clear on what tools you are using to produce the PDF. I downloaded the PDF
file you posted and the Document Properties indicated the PDF Producer was "Python PDF
Library - http://pybrary.net/pyPdf/";. The message below mentions a "known problem
with dip", and I don't know what "dip" is. Can you clarify how you are producing it
and how it relates to DocBook?

I think it's a known problem with dip. I asked about it on list about
6 months ago. Don't have the workaround handy.
