The problem is with FOP, the XSL-FO processor used by Oxygen. To be fair to
Oxygen,
FOP is free so it can be bundled, but it is not a finished product yet. When I
process
the FO file produced by the DocBook XSL stylesheets with XEP or Antenna House
XSL-FO
processors, the results are correct.
So FOP still has some bugs like this, and I don't know of any workaround for
this
problem.
That means to get good results, you will need to use an XSL-FO processor other
than
the free FOP. You can configure Oxygen to use a new XSL-FO processor once you
acquire
it.

the pdf was done with oXygen editor (fop 1.0, saxon). (I tried with
other fop and toolchains with the same result)

the "python pdf" library mentioned was only part of he program I used to
select some faulty pages of my 200 pages book.

it doesn't work in my case in some places ... very strange behavior: I
cannot find why in some places it works, and in others, not ...

I think it's a known problem with dip. I asked about it on list about
6 months ago. Don't have the workaround handy.
