Thanks for responding. Yes, the problem is with PDF. I was surprised, too. I 
thought that thead alone would take care of this and I have done no table 
customization.
I’ll have a developer here take a look and see if he can diagnose the problem.

It sounds like you're having trouble with PDF output. Is that correct?
I'm surprised that your headings are not repeated after a page break. I think 
that is the default behavior. Table breaking across pages is described here:

Do you have table customizations that might be affecting the way this works?
