Make sure you have an <index/> somewhere in your doc. 

If generating html, make sure generate.index is set to 1 (that's the
default)
http://docbook.sourceforge.net/release/xsl/current/doc/html/generate.ind
ex.html 

If generating html help and you're expecting an hhk file, see this
param:
http://docbook.sourceforge.net/release/xsl/current/doc/html/htmlhelp.use
.hhk.html (but you don't need an hhk for an index in html help).
