Fixed in r1934605 (2.4) and r1934606 (trunk).

Normalized all occurrences of "Server-Variables" and "server-variables" to
unhyphenated "server variables" in mod/mod_rewrite.xml and rewrite/flags.xml.
No RFC precedent exists for the hyphenated form (RFC 3875 uses "meta-variables"
for the CGI concept).

Also reworked the module summary to clarify that HTTP headers and timestamps
are a subset of server variables rather than separate categories, and linked
the term to its definition in the RewriteCond section.
