And a tiered labeling method using a separator, such as a hyphen, could be employed.

One example might be dialog-box title-subitem1-subitem2--for one employing three tiers. Another might be: File-command-subcommand-subsubcommand, etc.

If graphics were given easily recognisable names (for example numbered graphics being called Fig1, Fig2, etc., and inline graphics things like FooIcon), then it would be much easier to handle the "disappearing graphics" problem.
