Your actuator arm may be stuck, or the vacuum hose that goes to it may be kinked or cracked. The biggest job for this type of thing is generally getting to it itself (taking the dash apart).
Do you have the "automatic" heater control set, or the manual controls? I seem to recall some kind of electrical problem that somebody had last year with this same type of deal, that ended up being a wiring problem with his automatic climate control system (not the actual door/actuator).
