Also didn't know that the IAC actuator could fail and not show a code.  Good to 
have this list.

Cylinder misfires after start with good fuel pressure
are generally spark delivery problems, though it could also
be airflow.  Follow the manual...plugs and wires are the next route.

Your TB isn't sticking, is it?

Another possibility....crankshaft position sensor seems to be a
common failure point in LH series vehicles, and it would cause this
condition (so could camshaft position sensor, though it is less prone
to failure).  CKP sensor failure is usually indicated by the "cruise"
light flashing during misfire.

One final thing to check is timing...if the belt jumped a tooth it
would cause rough idle and multiple misfires.

The O2 sensor code is probably coming up due to the misfires.  It would
not be a factor in starting or idle while in open-loop mode.

will the engine start and run if you give it part throttle, but not  idle.
i had this on my 94 (3.3), was a badidle speed control motor.
did not show a code. 5 minutes to replace and no more  problem
