Summary: Changing then opening closed file causes inconsistency

Initial Comment:
Editing a file externally and then opening it in Eclipse (during a session) 
causes the new contents to be pre-pended to the existing contents on other 
peers, rather than containing the new contents only.

To reproduce:
- Start a session
- Edit and save file X on host in an external editor, ensuring that file X is 
closed in Eclipse
- Open file X on host and refresh
- Open file X on peer, you should see new contents then old contents

Message:
The refresh forces the DocumentProvider to reload the document, this
triggers a inputChanged (whatever) event which results in a text edit
activity (containing the whole new content). There is no error in the
logic, it is the fault of the user.
