Thanks.  I think I've straightened it out.  I had to add an outer element,
faq_document, and make changes to XSL to the XML to get it to work out.  One
thing to keep it mind is to keep as much of the UI-related stuff out of the XML.
That's why I moved the links at the top and bottom to the XSL file.  The
exception in this case is the content of the questions.

Although, if we were to do it right, we'd get rid of all the HTML and just name
things for what they are.  E.g., that keybinding HTML would become
<keybindings><keybinding/>...</>.  Same with the lists of examples.  We'd just
call them <lists>.  That's for another day.

Thanks for all your help.  I kinda took it as an opportunity to learn some more
about XSL (more than I bargained for).
