Perhaps you have a multi-step workflow set up? We have a two-step workflow, so two approvals are required before it goes live.

We created the Item (With Attachment) = OK 4. The already created item goes to Approval or Refusal = OK 5. When we approve the item, it does not approve and returns to the initial state of "approve or reject".
