This issue seems to have taken care of itself (after not working for 
days).  It suggests that there was some sort of problem with Creative 
Commons, and not with my setup.  What's frustrating is that, as I said, 
there where no errors in the console, the angular logs, nor the server 
logs.  The submission form just hung.

Anyway, it works now and if anybody else experiences a similar problem 
maybe just wait it out?
