It's not really my area of expertise, but aren't there files that are essentially links to media streams that would serve your purpose, if you just upload the "link file" as your bitstream, and give it an appropriate media type?
The W3C-recommended SMIL format appears to support this (.smil or .smi extension, media type application/smil+xml or application/smil). Some of the proprietary streaming protocols also specify their own link-file formats (such as .ram for RealAudio). I would think that if you use an appropriately constructued one of these as your bitstream, and make sure the media type gets reported correctly, it should look to your users just like any other kind of DSpace content, but start a stream going when it is downloaded.
