The Apache Portable Runtime provides IPv6 listening sockets by default, so it looks like it's logging your IP addresses in the IPv6 format. I'm not sure how APR was installed on your system, but if you build it from source, you can hand the ./configure script a flag of "--disable-ipv6" which turns it off completely.
