The problems sound like the high voltage power supply of monitors - either bad, weak or voltage drop on power source. Are units on UPS and all on same source? The swapped monitor showing same signs as replaced unit and multiple monitors/video cards would suggest power source problems - monitors less tolerant of low voltage then CPU PS. Any other problems with other devices running on same power source? Monitors running off outlets on CPU or direct connection to power source?
