Sounds like either improper SCSI Termination or two devices with the same
address.

Assuming that you've checked these two options, try booting with only one
device at a time and checking the resolved address with SCSI Probe
(incarnations exist at most ftp sites). Occasionally the cables that run
internally from the selector switch get disconnected and the devices don't
really have the address that you've set.

Termination is a bit trickier to figure out. Some devices have internal
termination (or a switch or turn it on/off), others require an inline FPT
(such as when connecting a desktop Mac to a PowerBook used in SCSI disk
mode). You might need to consult the manuals of the devices on the SCSI
chain to get a better idea of how each of them is handling termination.

A third thing to check (it has bit me in the behind before and is nearly
IMPOSSIBLE to find) is a bad cable, or incompatible cabling. I've ran into
problems when using both 50 wire and 25 wire SCSI cables in the same chain
(look to see if one of your cables is noticeably thinner or thicker than the
others).

SCSI is wonderful, but firewire is LONG OVERDUE (my $0.02)
