This sounds like what happened when people tried to edit the country file. USA QSOs were temporarily changed to "JZ0" prefix, which doesn't have states, so all states were lost if you saved your log. When you restarted DX4WIN, the QSOs would be put back to USA, but states would still be gone.
The only way to tell would to be to go back to an old log, perhaps in your 7.02 directory.
