This situation commonly occurs because of:
1. you actually have multiple callsigns in your log as I do
wn9vol, wa9vol, w9ol
of which each has its own start/end dates
or
2. you made an error on the certificate request
which would have not processed qso's that didn't fall within the range you 
entered.
(check the format you entered for the certificate)

but 1. is the most common.

if you did make an error in the certificate, do a 'renew' and you will get a 
corrected one
PS:, make sure your zones, grids are correct.
LoTW asks for ITU zone first and then CQ, which a lot of people didn't notice.
