[Dxbase] ICOM 756 Pro question

Anyone got one of these who can tell me what radio options to set in DXbase. 
Also, what address should be in the radio.

Got someone having difficulty and not sure what to do.  His radio reports it 
is using 64H but that seems wrong to me???
