Hi all I got a few helptul emails from Joe WA6AXE who has given me great guidance on how to proceed. The key fact is that LoTW doesn't use the prefix in matching records to awards, only the ARRL ID. The prefix is only used inside of DXBase, so it doesn't have to match anything else. Of course, its best to keep them in sync with the official DXCC list but it doesn't have to. There are already a few 'special' prefixes used inside DXBase so there will just be a couple more. I think the Updater can handle all of this okay, after you run the Initialize Tables command after the update.
The situation with the IOTA change for Curacao is going to be more difficult. RSGB has allocated a new IOTA group for Curacao, so any qsos with the entity after 10-10 will need to be manually changed (by you).
