It's the environment just before Production, an acceptance environment. I don't think anybody played with the folders but I cannot be sure 100%.
The repository is loaded with 260.000 documents (times 2 since they have a snapshot published in section). The problem occurs with some of the folders that have been created after the initial load. The data are stored in a DB2 database with JCR Bundle storage; the rest is local filesystem.
