If you can use cgi-scripts on your server, you can use "CGI2SPSS", which
is free for non-commercial use and available under
http://www.uni-jena.de/svw/metheval/projekte/evaluation/CGI2SPSS/

You can save client variables like IP-Adress and Browser type to scan for
duplicate responses.

I have worked with CGI2SPSS once and was very satisfied. If you set it up
correctly, you get your survey data on a silver platter :-)
