It's not a template, it's just common sense. How does anyone possibly expect people to fix problems WITHOUT all that information, at a bare minimum? Every time you make a bug report, you should include ALL information about your setup, hardware, software, anything special you've done, etc. If someone wants to put it up on the wiki, by all means.
