FB Don. Since you were responding to a threat specifically about the K3EXREF SMA I didn't want anyone to think that your comments applied in that case.
I, too, usually put a male connector on a female panel mount BNC to hold it while I tighten the inside nut. The SMA in question doesn't work like that. Why those connectors failed is a mystery.
