The formats are specified by the respective international treaties. For Canadian amateurs the format is specified by the "Convention Between the United States and Canada (TIAS No. 2508) Relating to the Operation by Citizens of Either Country of Certain Radio Equipment or Stations in the Other Country" (§97.107 (a)(1)). For amateurs from countries covered by CEPT (§97.5 (d)) or IARP (§97.5 (e)) the format is specified by those conventions.
