Is that the rule, or is this the rule (from the documentation): If the description is a file name or URL that points to an image, HTML export (*note HTML export::) will inline the image as a clickable button. If there is no description at all and the link points to an image, that image will be inlined into the exported HTML file. This latter rule would work. One would want the exported HTML to be a clickable button so that you could, for example, have a thumbnail on the page that points to a higher resolution version in a different directory. The thing that's missing from either approach is that you can't specify attributes -- especially the alt tag for creating w3c compliant, disability accessible, search-engine friendly image descriptions.
