additional effort on my end (I am a mailing list newbie, sorry!)...
Running 'emacs -Q' for my builtin org-mode leads so similar strange
behavior if I extend the entry's content to this:

Opening entry "B" with <Tab> leads to this result (screenshot):
Note that entry "C" has disappeared and "=Other text= appears
duplicated. Normal appearance comes back with "save file", "switch
buffer" operation or similar.
Still very strange... What else should I try?

I can confirm: Adding '#+STARTUP: overview' does not change the
observed behavior of the test file.
In addition I found that reducing the entry length for testing
purposes, brings back the expected fold/unfold display, e.g.

I wasn’t able to reproduce from your recipe, but I have been seeing
something similar when jumping to a folded headline with ‘imenu’. If
I can reproduce with ‘emacs -Q’ and a trimmed-down file I’ll post
here.
