Again, this is truly bizarre. I am a web designer for a company in Chicago, as well as running a freelance video editing business of my own. In short, I've had massive amounts of experience upgrading machines to OS X. And I have never needed to go through a complex ritual like the one you outline. This truly baffles me... it's gotta be a matter of user error; that's all I can think of.
