I do remember to quit it occasionally so that deletion takes place. Very occasionally have I received duplicate emails and as Chris says this may occur if there is a problem unpacking the mail. That is when I have seen it for sure but otherwise I don't see the problem, so I wonder if the problem may lie with your database or your mail hosting service as there seem few other choices.
