Summary: Not homing A axis causes program execution to hang

Initial Comment:
Not homing the A axis causes program execution to hang on a tool change with no
error message. The A axis is not used anywhere in the program. This can be
reproduced with the attached simulation configuration and g-code by performing
the following steps:

1) Get to machine on.
2) Home X, Y, and Z. Do not home A.
3) Open the attached g-code file.
4) Run the program.

Perhaps task should not allow transition to auto mode if all axis are not
homed. If coordinated motion is to be allowed on the axis that are homed, the
state of other axis not involved in the motion should not effect things.
