I implemented this in trunk.  It involved a lot of changes, so it
needs some testing.  Any help anyone can give is most appreciated.
Here is the commit message that describes the intended change.

     As discussed on emc-developers: wait for the spindle to be "at
     speed" (as shown by a hal input to motion) in these cases: before
     the first feed move after each spindle start or speed change;
     before the start of every chain of spindle-synchronized moves;
     and if in CSS mode, at every rapid->feed transition.

John T, can you please add this information to the docs wherever it
describes the inputs to motion or the spindle-to-motion interface?
