John, Try BABT or TUV. First, if you haven't already done so, you may want to get & review a copy of TBR 21 from ETSI. I believe that they still offer free copies on their website (below). Also, be aware that some countries have contributed additional Voluntary "advisory notes" in the ESTI document EG 201 121.
As far as the procedure goes, they will probably want to review technical documentation to familiarize themselves with your (Non-Voice analog) product. Then you would probably want to submit a sample of your product for a constructional review and explain the operation, features and options if any. At that point you will know if any modifications are needed before you submit for testing. Respond appropriately, next you should be advised on such things as; lab availability, # of test samples needed and required product & component documentation. PS.
