Are you possible referring to the IEC 417 symbol number 5172, which is the
symbol for double insulated equipment? This symbol is a square surrounded by
a larger square. This symbol and all of the IEC 417 symbols can be found on
the web at: http://w3.hike.te.chiba-u.ac.jp/iec417/ver2.0/html/index.html.
If you don't have access to the web, I can email the page with the 5172
symbol to you, just let me know.

I'm not really sure if this is what you are looking for, since I don't see
why it would be mentioned in a test procedure. It would however probably be
mentioned in the test report and the UL Follow-up Service Procedure for a
piece of double insulated equipment as these usually mention any symbols and
statements required to be on the equipment.
