Carmen, I'm not familiar with IEC 65, but for EN 60950 an attachment of a laser safety report to the EN 60950 report is typical. There is no requirement in IEC 825 (a.k.a. EN 60825)for use of a 'certified' laser lab. But as with many specifications, it is to your adavantage to use an independent 3-rd party laboratory, as there would less questions as to impartiality. Also note, that IEC 825 also applies to products that use LED-based optics, not just lasers.
For 21 CFR, you MUST file a report with the CDRH for laser products (no LEDs yet in the US). There is a special form that must be completed that forms the report. Additionally, the CDRH requires an 'annual report' which is to be received by them in September.
