Steve, According to CFR 47 Part 15 the label on the unit must contain the complete statement in your first paragraph. This requirement is in paragraph a(3) of section 15.19 "Labeling Requirements". This paragraph states that the device shall bear the statement in a conspicuous location on the device.
CFR 47 Part 15 also states that the statement in your second paragraph must be in the documentation. This requirement is stated in paragraph (a) of section 15.105 "Information to the User". This paragraph says "For a Class A digital device or peripheral, the instructions furnished the user shall include the following or similar statement, placed in a prominent location in the text of the manual." Then it has the statement in your paragraph. Hope this answers your questions.
