The replacement for BS5378 (which is the one I can always remember myself)
appears to be BS5499.
Here is an extract from the Foreword to BRITISH STANDARD BS 5499-1:2002
"Graphical symbols and signs - Safety signs, including fire safety signs -
Part 1: Specification for geometric shapes, colours and layout"
......
"This part of BS 5499 has been prepared by Subcommittee PH/8/1. It
supersedes BS 5499-1:1990, which is withdrawn.
BS 5499 covers safety signs and fire safety signs since it is considered
essential
that all safety signs follow the same basic vocabulary of safety colours and
geometric shapes.
BS 5499 comprises the following parts:
- Part 1: Specification for geometric shapes, colours and layout;
- Part 2: Specification for self-luminous fire safety signs;
- Part 3: Specification for internally-illuminated fire safety signs;
- Part 4: Code of practice for escape route signing;
- Part 5: Signs with specific safety meanings;
- Part 11: Water safety signs.
Part 6 on design principles for graphical symbols for use in safety signs is
being
drafted."
Part 1 states that it is aligned with ISO 3864 but it is not a dual (BS ISO)
or triple (BS EN ISO) numbered document, and you will have to look in the
other Parts for relevant references to international documents.
Most of the currently issued parts are dated 2002.

I hesitate to mention an old and withdrawn British Standard ( BS
5378:Part 3) but cannot immediately find a reference to a more recent
international version. this defines five sign layouts:
Prohibition signs - White background, circular red band and cross bar.
Warning signs - Background yellow, triangular band and text black.
Mandatory signs - Background blue, symbol or text white.
Safe condition signs - background green, symbol or text white.
Supplementary signs - Background white, text black
Looking through several catalogues, all the signs follow these patterns,
so it seems a good idea to follow this scheme.
