1. I think this is just a matter of preference. For IEC/EN testing, EFT mains
coupling requires 33nF capacitors while Surge mains coupling requires 9uF/18uF
caps and 0/10 Ohm resistors depending on coupling path (plus 1.5mH decoupling
inductors). Some manufacturers put it all in one box and some make separate
boxes. Some are manually operated and some operate automatically in
conjunction with the transient generator. Some users like the convenience of
having everything in one box, and others find it a bottleneck and worry that
if one thing breaks, the whole unit has to go back. Once you've verified with
the manufacturer that the coupling/decoupling component values are in
accordance with the standards, then it's just a matter of cost, reliability,
and personal preference. 
 
My [biased] personal opinion is that putting it all in one box requires
certain compromises on performance, component quality and reliability that
we're not willing to make. As they say, your mileage may vary.
 
2. Yes, you should be able to use Lx-N, Lx-PE and N-PE on a three phase CDN
just as you would use L-N, L-PE and N-PE on a single phase CDN.

We are about to make a purchase of surge and EFT test equipment, and I have
a couple of questions about coupling-decoupling networks:

1. Why do some test equipment companies sell different CDN's for surge than
for EFT, while other companies manage to put out single designs usable for
both?

2. Can 3-phase CDN's be used for single-phase equipment, or do we need to 2
CDN's?
