I suggest that you go to the System Safety Society webpage
(http://www.system-safety.org/) and start there.
You can find a local chapter nearby that may be able to assist you.
A product for the US Government should use MIL-STD-882D ("System Safety
Program Requirements/Standard Practice for System Safety")

This should be specified in the contract. If you are designing something on
speculation, then you should get this standard and apply it.
Be advised, this standard is not a prescriptive engineering standard but a
safety management standard.
>From there, it is up to the contractor to determine which specific hazard
analysis techniques (FMEA, Fault Tree, Human Error Analysis, Energy Trace &
Barrier Analysis (ETBA) or one of over one-hundred different analysis
techniques that are to be applied. (Reference the System Safety Society's
"System Safety Analysis Handbook, 2nd Ed. 1997")
When the potential hazards are identified and ranked according to severity
and probability of occurrence, they are given a hazard ranking.
This ranking then determines whether, and what kind of, mitigation will be
used to minimize the identified risk.
There are companies that specialize in performing these analyses in
accordance with MIL-STD-882.

I'm looking into what is required in carrying out a Hazard Analysis on a
small, battery powered, radio to MIL-HDBK-454.
Would anyone care to indicate the size/level of detail typically required
or point me in the right direction with examples.
