For boards with planes on the outside layers, surface mount components
especially will cause problems, as the planes need to be cleared away to
make pads for soldering. If the components are closely spaced, or if the
component itself is long (a large IC or connector), there can easily be a
large slot in the plane.
The other thing I have seen is a desperation move by the PC designer who is
running out of room to run traces. It is very tempting to run a trace on a
plane layer, which then causes the plane to be to be cut.

What are some of the causes that create breaks or slots in ground or power
planes in multilayer boards? Voltage partitioning comes to mind and closely
spaced vias. Others?
