Hi all – I have before me a FDA import form 0910-0025
At the bottom there are two boxes: One - Signature of importer of record Two – Name and Title of Responsible person. I have looed on the FDA web site and cannot find (yet) any guidance as to who these folks are. Soea any one have experience with these forms?
